The Senior Hub-Meals on Wheels of Fairfield County is seeking a Care Coordinator who possesses strong interpersonal communication, time management, and organizational skills. In this role, you will assist older adults by assessing their needs and aligning them with services provided through our agency.
The Senior Hub-Meals on Wheels of Fairfield County is a large, 501c3 nonprofit organization serving Fairfield County. The agency serves as the county’s office on aging providing programs and services to improve health, connection, and independent living for those 60 and older living in Fairfield County.
ESSENTIAL JOB FUNCTIONS
• Perform in-home, phone, and walk-in assessments of older adults to identify needs
• Provide information and referral services to older adults and/or their family members
• Perform case management and appropriate documentation
• Assist older adults with problem solving related to services
• Complete and maintain proper certifications and CEUs
REQUIREMENTS
Associate’s degree in social services or related program, preferred
A minimum of 2 years’ experience in human services, geriatrics, or health care setting or similar role
Hold a valid driver’s license and good driving record
Active vehicle insurance
